Students who are interested in having their character be on the Quidditch team may add a section, under the "Magic" header, for their character's desired position.
Note that first years are not accepted to the team.

Basics
Name: First Name & Last Name Required.
Age:
Please be realistic. Anywhere from 10- 18 is acceptable. 20 & up for professors or school staff. Bear in mind that, for professors, they undergo years of training and school before they become a professor - just as in real life.
For students, age should correspond to year:
First year - 11, Second Year - 12, etc.
Gender:
Self-explanatory.
Desired House/Staff Position:
Gryffindor, Ravenclaw, Slytherin or Hufflepuff. Or if you are applying staff, one of the free positions.
Year: First through to seventh.
Race: Caucasian, African American, etc; A blend of several is acceptable.
Weight/Height: (optional).
Pet (If Applicable):
Students may bring a cat OR an owl OR a toad.
All other choices of pet must be approved by staff members.
Magic
Wand Type ( Wood type, length ( Wands average 12 & 1/2" and below. ), Wand Core.
Example: Elm and Griffin Feather. 10 1'2"

If an Animagus - which you need staff permission to be - it will generally be the same as the Patronus.

Make sure it's a country and not a continent or region of a country. Feel free to add the specific area.

Pureblood, muggle, half blood or mixed blood. ( Preferably one of the first three. )
History:
( At least two paragraphs. And not just: John was born in England. John lived a good childhood. John had lots of friends. John's parents sucked. Please put some actual effort into it - a character's history is what defines who they are, and everyone should be in SOME way different. )
